999 Call 999 in life threatening situations where emergency care is needed (available 24/7)
NHS 111 Call 111 if Urgent Care is needed, but this is not an emergency (available 24/7)
Devon Care Coordination Service Call 01752 422 734, 8am – 8pm, seven days a week.

 

The Devon Care Coordination Service can provide Care Home staff with support for any Care Home residents. If you are unsure about calling 999 or 111, an Advanced Clinical Practitioner or General Practitioner will support staff through a clinical conversation to identify an alternative pathway to an ambulance and hospital admission, for example support in the community or direct access to a hospital service.

Devon Urgent Care Response (UCR) Service Call the local service as follows, 8am – 8pm, seven days a week.

The UCR Service is delivered by locality-based teams of healthcare professionals across Devon and provides urgent care, including assessments and provision of equipment as required, to people in their place of residence (this includes Care Home residents). The service provides a rapid response within two hours of referral and can provide support for Falls; Frailty; Mobility issues; Confusion / Delerium; Palliative / End of Life Care, Unpaid Carer breakdown; and Urgent equipment provision.

For routine and / or ongoing health problems, contact the GP Practice during surgery hours.  Out of Hours urgent GP services are accessed through NHS 111.
